These are the main components in the stuffing box of the hydrant. The very top item is the umbrella operating nut. Directly below that is the dome. Below the dome, is the packing gland or follower (smaller red part) that is attached by the two follower bolts. The packing gland/follower slides down into the packing plate which is attached to the head of the hydrant. Below the packing plate is the seal which goes between the packing plate and the hydrant head. Behind the seal is the bronze sleeve that the stem slides into.

Below are images illustrating the hydrant after taking the umbrella operating nut and dome off the hydrant. In picture to the left, the elliptical shaped part in the center is the packing gland or follower. The four packing plate hold down bolts are visible. To the right is a side view of the same parts. The packing gland/follower is bolted into the packing plate.
